Pre-Requisite Form for the Pre-Vendorization Orientation

If you are an Agency interested in becoming a Vendor (Service Provider) with the Eastern Los Angeles Regional Center and you would like to attend our Pre-Vendorization Orientation on September 18th and 9th, 2024, from 8:30am to 12:00pm, please fill out this Pre-Requisite form.
