ELARC continues to be committed to person centered practices through organizational change, as well as promoting person centered tools within the community. 

Health and Human Services Guiding Principles

As part of our commitment to the philosophy of person centered practices, ELARC continues to build on person centered thinking skills and approaches to help ensure that the individuals we support are at the center of creating a life of meaning and purpose. We also strive to provide families, providers and our community  a shared understanding of these principles through training and resources.

The California Department of Health and Human Services (CHHS) oversees departments and state entities that support children, individuals with disabilities, seniors, and other unique individuals.  CHHS works in partnership with the Department of Developmental Disabilities and the Regional Centers. Person Centered Planning Virtual Workshop:

The Person Centered Planning Workshop is to help you learn more about the foundational elements of any person centered plan as well as examples of different plan types. The workshop is designed to provide a clearer understanding of why person centered plans are important, as well as provide participants an opportunity to clearly define the foundational information that should be an any person centered plan. Workshop attendees will then be able to expand on that information in the person centered plans that they choose to later develop.

Your IPP and Person Centered Practices

Ahead of your Individual Program Plan (IPP) meeting, your service coordinator should mail you a PCT one-page profile. Completing this one-page profile helps you prepare for a person centered approach to the meeting and helps the Service Coordinator in finalizing an IPP that is person centered. The one page profiles are provided below in these languages:

  • HCBS Final Rule and How it is Supported by Person-Centered Planning: This training was provided in September by the California Department of Developmental Services (DDS), in partnership with Public Consulting Group and Support Development Associates. This was an introduction to person-centered thinking, and a review of the person-centered service planning requirements specified in the HCBS Final Rule. View the PowerPoint presentation.
