The one page profile workshop is for Self Determination participants and their circle of support, to learn more about the foundational elements of any person centered plan as well as examples of different plan types.
The workshop is designed to provide a clearer understanding of why person centered plans are important, as well as provide participants an opportunity to clearly define the foundational information that should be on any person centered plan. Workshop attendees will then be able to expand on that information in the person centered plans that they choose to later develop.
This workshop is now opened to all families served by ELARC and vendors; however, please notice that we have limited space and our Self Determination Participants will get priority seating. RSVP now before space fills up.
