This session will be held from 10:00 am to 12:00 pm in the Boardroom at the ELARC Alhambra office located at 1000 South Fremont Ave. in Alhambra.
Please bring your seven digit "UCI" number to the session (available on your IPP or by calling your Service Coordinator)
The law requires that before the Self Determination program can be made available, the Department of Developmental Services (DDS) obtain approval for federal funding. DDS has submitted the application for federal funding and it is anticipated there will be approval by the end of this calendar year. Once that application is approved, it will allow us to launch Self Determination in California.

Please be prepared to provide your/your family member’s Unique Client Identifier (UCI) number when RSVP-ing or on the day of the meeting. You can find your UCI number on your IPP or other documents from ELARC or contact your Service Coordinator who can provide the number to you.
This session is in English. If you wish to participate in a language other than English, please contact Elisa Salazar at (626) 299-4772 two weeks prior to the meeting to allow sufficient time for ELARC to make every attempt to meet your request.
